At the conclusion of the survey you will be asked if you would like to be entered into a random drawing on June 1st, 2012 to win a free UNC Charlotte parking pass for the 2012-2013 academic year. If you elect to enter the drawing, your contact information will not be associated in any way with your responses to the original survey.

Are you currently in recovery from alcohol abuse or any other kind of substance abuse?
